The Change Order will increase the construction contract amount by $76,967.00 for a total construction contract amount of $2,192,146.89.

2. Consideration and approval to authorize the County Judge to sign this Change Order.
 
E. 67368   "Subject to: HB 1295 Compliance.” Requesting authority to purchase 2 large trays through Hidalgo County's membership/participation with Buyboard awarded vendor, Demco, Inc. (contract # 487-15). The amount of purchase is $360.79 (quote attached) funded thru UCP FY 30 (2017) Mercedes Public Service (General) Library funds.
 
15.   Health & Human Services Department:
 
A. 67424   1. Requesting approval of the Certification of Revenue in the amount of $71,384.01 as certified by County Auditor. Funds are program income generated from the Medicaid Administrative Claiming program.
2. Requesting approval of the budget appropriation in the amount of $71,384.01.
 
B. 67443   1. Approval to rescind approval of item 16, A, 1 (AI-67209) on 10/23/2018 due to changes in transfer and settlement dates.

2. Discussion, consideration and approval to draw down funds for the Uniform Hospital Rate Increase Program in the amount to be determined by Health and Human Services Commission (HHSC) instructions from the Local Provider Participation Fund (LPPF) with an IGT transfer date of 11/12/18 and settlement date of 11/13/18.

3. Approval of Certification of Revenues as certified by the County Auditor from the Local Provider Participation Fund (LPPF) in the amount to be determined by HHSC instructions.

4. Approval of 2018 appropriation of funds from the LPPF in the amount to be determined by HHSC.
 
C. 67433   Requesting approval for County Judge to sign the Federal Funding Accountability and Transparency Act Certification as part of the Child Health FY 19 grant contract amendment accepted and approved on 09/04/18 through AI-66400.
 
D. 67415   Requesting approval to accept the field support from Centers for Disease Control for fiscal year 2019. The purpose of the field support is provide local health departments assistance with ZIKA related activities.
